Office Policies

Will I receive a reminder about my appointment?

If you have provided your e-mail, we will send you an e-mail reminder four calendar days prior to your appointment. If you have provided your cell phone number, we will send you a text reminder, two calendar days prior. If you have provided both your e-mail and cell phone number, you will receive both reminders.

 

How do I cancel or reschedule an appointment?

There are two options to cancel or reschedule your appointment. If you were sent an e-mail reminder, you can click on the red cancellation link. This will remove your appointment from the schedule and will notify our staff to contact you to reschedule. You can also call the office at 808-947-5606. Remember that we have set aside time for your visit which may prevent others from being seen that day. You may be assessed a $25.00 no-show fee if you fail to keep your appointment or cancel within 24 hours of your scheduled time.

 

What time do I need to check-in for my appointment?

Please check-in 15 minutes prior to your scheduled appointment. If you are a new patient, please check-in 30 minutes prior to your scheduled appointment. Upon check-in, please complete a patient update form and present your ID and insurance card(s). If your appointment is for an annual exam, please bring a list of your current medications.

 

Is it possible to designate someone to call on my behalf?

Yes, you will need to complete an Authorization for Release of Information form.

 

How do I request copies of my medical records?

You will need to complete a Request for Medical Records form. If it is being sent directly to another physician, there is no charge. Otherwise, please call 808-947-5606 to obtain fees and time frame to obtain records. Please allow 3-4 weeks to process.

 

Medical Concerns

How do I refill a prescription?

If there are refills available, it will be indicated on the bottom of your prescription label. Simply call the pharmacy phone number listed on the prescription label. Most pharmacies have an automated refill line that you’ll just need to enter the prescription number from the label. If there are no refills, please call the office at 808-947-5606. If you’re not sure if you have a refill, please call the pharmacy to verify. If you are due for your annual exam, we will need to make an appointment to fill a new prescription.

 

What is my copayment or plan benefits?

Copayments and plan benefits vary depending on your insurance. Your insurance company would be the best resource to obtain copayments and benefits of your specific plan. 

 

Can I keep my Annual Exam appointment if I have my period?

If you are comfortable having an exam and your period is light, it is okay to keep your appointment. If your period is heaving (soaking a pad every hour or having to change your pad or tampon every 2 hours), we will need to reschedule your appointment as it may cause a false reading.

 

How often do Pap Smears need to be done?

Routine pap smear screening is recommended starting at age 21. Over the age of 30, your doctor may recommend a pap smear every three years depending on your age and history. Pap smear guidelines are constantly changing so your doctor will discuss at your annual exam. HPV screening is also part of the pap smear screening and may be ordered based on current guidelines.

 

How often do Annual Exams (Well Woman Exam) need to be done?

Although you may not be due for a pap smear, you should still be seen annually for your pelvic and clinical breast exam.

 

How often do mammograms need to be done?

The American College of Obstetrics and Gynecology (ACOG) recommends:

Ages 20-39: Clinical breast exam every one to three years. This is usually done at your annual Well Woman Exam (Annual Exam).

Age 40 and older: Mammography screening every one to two years. Clinical breast exam should be done every year. This is usually done at your Annual Exam (Well Woman Exam).
